Meet Doctor Kinga Koprowicz – Your Trusted Marine Medical Examiner (MME)
- All medications, prescriptions and over the counter medications used in the past 12 months.
- Driver’s license and Health Card (for identification purposes).
- Complete page 1 of the Medical Form and bring it to your appointment.
- Summary of your past medical history; use page 2 of the Medical Form for a guide on questions that will be asked.
- Last vision prescription/examination from your optometrist/ophthalmologist.
- Your eyeglasses or contact lenses will have to be removed for the purpose of the examination.
- Your current Medical Certificate (if applicable) for renewal purposes.
- ECG, audiogram, urine and blood testing is available during the visit if required for the purpose of the examination.

For Seafarers with Diabetes:
- The stability of your diabetes
- Glycated hemoglobin
- List of hypoglycemic episodes in the past 12 months
- List of cardiovascular, ocular or neurologic complications of your diabetes
For each Medical Examination submission, please provide your glucose self-monitoring log from the previous month along with a detailed report from the healthcare provider treating your diabetes. This report should include:
New Applicants and Renewals: Successful applicants will receive a temporary certificate at the end of the examination.
